You need to have a medical examination before you can apply for your provisional licence. DVLA in Swansea will provide you with a D2 information pack containing a D4 form which you should bring along to the medical examination. The D2 information pack can be ordered on line >
LGV licence renewals
Existing lorry drivers are required by the DVLA to have a medical at the age of 45 and then every five years until the age of 65. After 65, the medical has to be done every year. DVLA should send out a reminder letter, together with the D4 medical form a few weeks before your medical is due. If you need a D4 form then you can download one from the DVLA.
The vision regulations have changed for professional drivers who wear spectacles (not contact lenses) when driving. You now have to bring along to the medical assessment an opticians prescription which relates to the spectacles you are wearing. It is no longer a requirement for this prescription to be dated within 4 months of the medical. If you do not have one you can see an optician after the medical who will complete the first page of the D4 form.
The visual acuity criteria, which are measured during the medical assessment, have also changed. The better eye must have slightly improved vision (6/7.5) than before but the worst eye can now have much poorer vision (6/60) than before the changes.
Specific medical conditions
If you have ever had ischaemic heart disease (a heart attack, angina, coronary artery bypass graft, or insertion of a stent) you must have a treadmill test every three years. If you have had a treadmill test in the last three years you should be able to get a copy of the report from your doctor, otherwise the DVLA will organise this hospital test for you following your medical.
If you have diabetes and use insulin you may be able to hold an LGV/PCV licence but you will need to be seeing a diabetes specialist every year and keeping a record of at least twice daily blood sugar tests.
